New Patients,  please call our scheduling desk at 303.469.7300 in the Denver area or fill out the form below.

New patients, please download and complete the New Patient Registration Form and bring it with you to your appointment.

If you are an existing patient and wish to reschedule, please contact our office by phone only.

Please allow 1 hour & 15 minutes in our office for your initial appointment:

  • It will take about 10 to 20 minutes to fill out your paperwork.
  • You will then spend about 20 minutes with a New Patient Intake Specialist, who will explain our procedures to you and answer any questions you may have.
  • The Doctor will then examine you and gain a through understanding of your condition.  He will then explain how he can help you, answer any questions you may have and proceed to treatment.

    We have found that the amount of time it takes to verify insurance is somewhat variable.  As this process can significantly lengthen your appointment and cause scheduling problems, our office policy is to verify the patient's insurance prior to their visit.  If you wish to have your insurance verified prior to scheduling your appointment, please call our scheduling representative in Denver at (303) 469-7300 or Colorado Springs at (719) 634-6700.  They will be happy to assist you with this.

    Our policy is to review your insurance benefits prior to your appointment so that your time in our office is as swift and possible and our attention is focused on getting you well rather than administrative matters.  If you prefer to wait until your visit to have our staff verify your benefits, we can accommodate you, however due the length of time we may spend on hold with your insurance company, this may greatly lengthen your visit and may even delay treatment.  When we call you to confirm your appointment time, we will obtain your insurance information for the purpose of making out patients' time spent in our office as efficient as possible.
